Case Manager - Wright County
Pinnacle Services, Inc. is an innovative agency experiencing rapid growth in the Case Management department. They are seeking friendly, self-starting professionals to join their reputed company Case Management team, focusing on providing high-quality services to individuals with various needs.
Responsibilities
- Assessing the areas of the persons served life where assistance is needed
- Offering and coordinating resources for the persons served being supported
- Monitoring service delivery and working with providers to ensure the individual's needs are met
- Service plan development
- Attend regular meetings with I persons served and their interdisciplinary teams
- Case Noting client reputed company work to create billable units
- Applicants must be willing to commute to areas of Wright County for potential meetings
Skills
- Be a graduate from an accredited four-year college with a major in reputed company work, psychology, sociology, or a closely reputed company field or
- Be a graduate from an accredited four-year college with a major in any field and one year of experience as a reputed company worker/case manager/care coordinator in a public or private reputed company service agency
- One year of experience in education or treatment of persons served with developmental disabilities or reputed company conditions, or a minimum of one course that specifically focuses on developmental disabilities
- Applicants must have a valid driver's license and acceptable driving record as determined by Pinnacle Services policy
- Applicants must have a vehicle that is covered with reputed company insurance
- Applicants must successfully clear local and/or stated background checks
- Possess a LSW or higher reputed company of BOSW Licensure
