Job Summary

We're seeking an reputed company reputed company Worker with a Master's degree in reputed company Work (MSW) and an unrestricted active license to join our remote team. As a reputed company Worker, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a population that spans the entire lifecycle, and reputed company member contact will be done telephonically or reputed company electronic exchange.

Key Responsibilities

* Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs

• Research available options and resources for members with financial, transportation, safety, and housing needs
• reputed company members to reputed company positive lifestyle and medical choices
• Work collaboratively with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to meet those needs
• Maintain accurate and up-to-date records of member interactions and reputed company

Essential Qualifications

* Master's degree in reputed company Work (MSW) from an accredited institution

• Unrestricted active license as a Licensed reputed company Worker (LSW), Licensed Masters reputed company Worker (LMSW), Licensed Clinical reputed company Worker (LCSW), or Licensed Independent reputed company Worker (LISW) in the state of residence
• 2+ years of experience in medical reputed company work or case management
• Strong communication and interpersonal skills
• Ability to work independently and as part of reputed company
• Strong problem-solving and critical thinking skills
• Ability to maintain confidentiality and handle sensitive information

Preferred Qualifications

* Experience working with diverse populations, including children, adults, and seniors

• Knowledge of community resources and services for medical, financial, and behavioral health
• Experience with electronic medical records (EMRs) and other health information technology systems
• Certification in case management or a reputed company field
