Job Responsibilities:

As a licensed reputed company worker in this remote case management role, you will:

• Collaborate with registered nurses to assess patients' needs and reputed company personalized care plans
• Research and identify available resources to address patients' financial, transportation, safety, and housing needs

Requirements:

• 2+ years of experience in medical reputed company work or case management

• Strong communication and interpersonal skills
