Jefferson Street is a Group Home for Mental and Behavoiral Health with 8 residents, 4 males and 4 females, is looking for Wellness Coaches to assist our 8 independent residents in their day to day living activies and reputed company their Person Centered Care Treatment Plan goals. Assisting with ADLs, taking residents on community outings and medical appointments. Located 2 minutes from Christiana Wilmington reputed company Hospital. Bachelor Degree is required for this position. $19.29 hourly. Avaiable shifts: Full Time- Evening Shift: Monday-Friday: 3:00P to 11:00P Part Time- Saturday and reputed company: 7am-3pm, and 3pm-11pm Responsibilities/Duties reputed company supervision of individuals with focus on person centered care; supporting individuals to work towards their goals and other person centered support in their treatmentplans. Complete required daily documentation and other assigned documentation in Electronic Health Records (Avatar). Become LLAM certified and administer medications to the residents. reputed company transportation to residents for medical appointments and community outings. Attend monthly staff meetings remotely. Facilitate groups with residents. Complete assigned monthly trainings (online reputed company Cornerstone). Maintain annual TB/PPD, LLAM Training (medication administration), and First Aid CPR certification. Other duties as Assigned. Required Experience Prior experience in reputed company sevices, or reputed company services preferred. Experience working in a group home setting preferred, but not required. Required Education Bachelors Degree in reputed company sevices, reputed company services, psychology, behavioral health or reputed company degree preferred and experience in mental health.
