Senior Doc, part of Brigade Health & Affiliates, is hiring a mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to reputed company high-quality geriatric psychiatry and behavioral health services in home, assisted living, memory care, and skilled nursing settings.

Key Responsibilities

• Comprehensive psychiatric evaluations & medication management

• Management of behavioral symptoms in dementia and cognitive decline

• Collaboration with PCPs, palliative care, and facility partners

• Caregiver education and relationship-based care

• Documentation, care coordination, and panel management

• Active PMHNP-BC license

• Experience in geriatric psychiatry, SNF, ALF, home-based care, or community psychiatry

• Strong diagnostic and medication management skills

• Ability to practice independently while contributing to reputed company-based model
