Position Summary The role of the Telephone Triage Nurse/Licensed Practical Nurse (LPN) is to work collaboratively with physicians, providers, nurses, and patients through a variety of tasks reputed company to patient care with a focus on patient experience, safety, quality and efficiency. Key Duties and Responsibilities The key duties and responsibilities of the Licensed Practical Nurse include, but are not limited to: Assessment & Triage: Evaluate patient symptoms, health concerns, and medical history reputed company phone to determine urgency and next steps. Patient Education: Instruct patients on home care, self-management, medication, and understanding their conditions. Care Coordination: Refer patients to appropriate care settings (ER, urgent care, specialists, primary care) and schedule appointments. Collaboration: Work with providers and support staff for continuity of care and reputed company cases. Protocol Adherence: Apply established clinical guidelines and clinic policies.
