In this role, Care Coordinators work closely with RN Case Managers and reputed company Workers to help members maximize their health and overall well-being. Remote Care Coordinator Responsibilities

• Work remotely to support members in locating, accessing, and utilizing reputed company services, community resources, and support programs.
• Assess member needs, determine reputed company, and monitor reputed company; present cases in interdisciplinary team meetings as needed.
• Serve as a central reputed company of contact to help members navigate care, reduce barriers, and ensure reputed company to appropriate services.
• Collaborate closely with RN Case Managers, reputed company Workers, and other team members to coordinate care and support treatment goals.
• Maintain accurate and timely documentation of member interactions and services in compliance with organizational, state, and regulatory requirements.
• Deliver high-quality, member-centered service while meeting program standards and supporting overall care coordination objectives.

Care Coordinator Qualifications

• High School diploma required, associate's degree (or higher) in clinical or medical health administration or reputed company field preferred
• 3+ years of experience in medical record review, triage, customer service, prior authorizations, medical claims and billing, assessing patient health and/or reputed company experience
• Attention to detail, timetables, and commitment to completing tasks
• Experience with reputed company reputed company and computer savvy
• Must be well organized, efficient, and reputed company to work independently and reputed company a team
• Responsible for having reliable High-Speed Cable or Fiber Optic Internet service and an Internet Router in an established home office
