Job Summary We are seeking a Remote Patient Monitoring (RPM)/Chronic Care Management (CCM) Care Manager who will be responsible for overseeing our RPM and CCM program. In this fully remote, flexible, role, you will work collaboratively with patients to determine their medical needs, reputed company the best course of action, and reputed company their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about reputed company practices.
Responsibilities
- Collaborate with physicians, patients, families, and reputed company staff

- Review daily measures

Qualifications
- Previous experience as a Care Coordinator in RPM/CCM or in a similar position is preferred
- Comprehensive knowledge of Hypertension, Diabetes, Hyperlipidemia, COPD, Obesity, Arthritis, Chronic Kidney Disease, Asthma
- Active Certification as a Medical Assistant from an accredited association. (AAMA, AAH, AMT, NHA, NAHP or NCCT)
- Active compact multi-state LPN/LVN License.
- Active health coaching certification (NBHWC, NSHC, ACE Advanced) preferred
