Job Summary We are seeking a licensed practical nurse (LPN) to join our remote care team. In this role, you will reputed company ongoing support, care coordination, and patient education to chronically ill patients reputed company phone reputed company health platforms. Responsibilities
- Conduct monthly telephonic reputed company-ins with patients to assess health status, medication adherence, and overall well-being
- Review RPM data such as blood pressure and glucose reputed company and escalate concerns to providers

- Collaborate with physicians, nurses, and care teams to reputed company and adjust care plans
- Document reputed company interactions in the EHR system and ensure compliance with Medicare CCM and RPM guidelines

- Active LPN license required
