This is a remote part time position in the Georgia area but may require visits to one of our closest reputed company Offices in Georgia. Candidate MUST reputed company in the state of Georgia.

Overview

The Case Manager, RN is responsible for conducting initial in-home assessments, orientations for new patients and caregivers, and providing ongoing care management and support. This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care. The Case Manager, RN collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, reputed company individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health reputed company, safety, and regulatory compliance. The Case Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely reputed company, effective care coordination, and reputed company patient reputed company.

Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation, ensuring that reputed company required forms and standard procedures are followed.
• Develops, implements, manages patient's plan of care

Education and Experience

• Diploma from an accreditted nursing program. New hires welcome!

Is a Plus:

• Home care case management
• Care coordination experience

Qualifications

• Valid and Active Registered Nursing License issued by the Georgia Board of Nursing
• Ability to reputed company cases through effective summaries.
• Ability and flexibility to travel (usually by car) reputed company assigned geographic area
• valid driver's license and car insurance required.
• Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office) with the ability to teach others.
