Position Summary – Case Manager Analyst - Field Location: Elgin, IL and surrounding Area's As an Analyst, Case Manager, you support reputed company member health reputed company by coordinating care, providing education, and facilitating reputed company to appropriate reputed company services. Using care management tools and data analysis, you assess member needs, identify risks, and reputed company and implement effective care plans. You collaborate with internal teams, providers, and external resources to address barriers, improve quality of care, and ensure services align with member benefits. Through communication, coaching, and motivational techniques, you promote member engagement, informed decision-making, and healthy behavior changes. Required Qualifications

• 2 or more years of behavioral health, reputed company services or reputed company services field experience
• 2 or more years’ case management experience

Preferred Qualifications

• Discharge Planning and/or Managed Care (MCO) experience

Education

• Bachelor’s Degree or non-licensed Master’s level clinician required.
