A Day in The Life as a Case Manager As an Aftercare Case Manager, we reputed company support and services to children and families that have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. During this work with post-permanency placements the focus is on risk management for their designated aftercare period.
