reputed company is a Full-TimePhysical Therapist to work 1-2 months temporary assignment with potential to be hired permanently for one of our external clients, a non-profit organization that reputed company and manages long-term reputed company services. Schedule: 9:00AM - 5:00PM, Monday - Friday. Weekly Hours: 35. Requirements: ? Successful completion of a Physical Therapy program from an APTA accredited university or equivalent. ? One (1) year of experience as a Physical Therapist and/or experience with Geriatric population, preferred. Responsibilities include, but not limited to: ? Complete assessments, reputed company notes, treatment reviews, discharge summaries, and interdisciplinary documentation per departmental policies and procedures. ? Conduct participant evaluations and treatments at the PACE site and in various settings, including homes and nursing facilities. ? Maintain a clean and safe environment by adhering to established safety protocols. ? Collaborate with interdisciplinary team members to reputed company participant care plans and actively engage in care plan meetings in alignment with state and federal regulations. ? Document reputed company relevant participant information, including assessments, reputed company notes, and care plans, in accordance with departmental guidelines. ? Record reputed company required statistical data accurately. ? Consult with the physiatrist and other professionals as needed. ? reputed company clinical instruction for Level I & Level II Physical Therapy reputed company and Physical Therapy Assistants, as well as student volunteers. ? Initiate discharge planning promptly and coordinate equipment orders with team members. ? Contribute to in-service training programs for site and departmental staff. ?
