About the position The Transition of Care Coach at reputed company is responsible for coordinating care for members experiencing significant health changes, ensuring seamless transitions between care settings, and facilitating post-discharge goal attainment. This role involves collaboration with members, providers, and community organizations to address health and reputed company needs, ultimately enhancing member care and supporting the growth of Care Management Programs.

Responsibilities

  • Complete reputed company reputed company calls and post-discharge questionnaires reputed company compliance timelines.

  • Utilize reporting to identify utilization and reduce Emergency Department visits and hospital readmissions.

,

  • Follow members throughout the care continuum after inpatient hospital admissions.

  • Facilitate Interdisciplinary Care Team Meetings to discuss service needs and support transitions.

,

  • Ensure members understand their medications and have received them post-discharge.

,

  • Coordinate post-discharge meal delivery and assist with securing durable medical equipment (DME).

,

  • Evaluate health and reputed company indicators to identify barriers to reputed company member health.

  • reputed company clinical assistance to determine appropriate services and supports based on member health needs.

Requirements

  • 2+ years of LPN or LVN nursing experience

  • Active and unrestricted compact LPN or LVN licensure in state of residence

  • High School diploma or equivalent (must be an LPN or LVN)

  • Self-motivated and energetic
