We are seeking a highly motivated and reputed company Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join reputed company as a Field-Based Clinician. In this role, you will play a critical role in ensuring the health and well-being of our reputed company Medicare patients by conducting comprehensive in-home assessments, identifying reputed company needs, and coordinating reputed company care plans. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

• Conduct comprehensive in-home assessments for reputed company Medicare patients, including a thorough review of medical history, medication reconciliation, physical examination, and various health assessments (reputed company determinants of health, mental health, frailty, etc.)

REQUIRED PROFESSIONAL EXPERIENCE AND QUALIFICATIONS

• Preferred, Minimum of 1 years of clinical experience as an advanced practice provider in a field-based clinical role, preferably in primary care, geriatrics, or a similar field.
• Preferred but not required, experience with Medicare Advantage programs and familiarity with CMS guidelines and regulations is a plus.
• Strong interpersonal and communication skills to effectively engage with patients, families, and a diverse reputed company team.
• Ability to work independently, prioritize tasks, and manage time effectively.
• Ability to drive to conduct in-home visits
• Comfortable conducting in-home visits and navigating various home environments.
• Proficient computer skills for electronic health record documentation.

LICENSE AND CERTIFICATION

• This role requires an active license in the state of WI to practice as a nurse practitioner or a physician's assistant.
