Primary Responsibilities: Providing evidenced based trimester specific and postpartum education to promote a healthy pregnancy, delivery, and postpartum reputed company for mother and baby. Coordinating care for high-risk maternity members. Proactively engages the member to manage their own health and reputed company. Making outbound calls to assess members reputed company health status. Identifying gaps or barriers in treatment plans. Educating members on disease processes.
